Education Program Booking Form
This online application is the first step to complete your Education Program booking for the 2011-2012 school year. If you prefer to fax or mail your application please use our printer-friendly PDF form (45 KB). After you submit your application, you will receive an email receipt of your submission. Our staff will then determine if your request is feasible and then you will receive a reservation confirmation by e-mail within one business day. We will contact you by email if we need more information to process your booking.
After you receive your reservation, review it carefully to ensure that the information is correct. To confirm or change your booking please reply to your emailed confirmation with either status in the subject line and any changed information in the body.
Cancellations must be received, by email to programs@greyroots.com or by phone to the Education Liaison 519 376 5140 ext 6128, 48 hours in advance of your scheduled visit. Failure to confirm your reservation is not considered a cancellation.
Cost per attendee is $5.00 plus HST per student per program. A 15 student minimum is required for education programming. Teachers and Education Assistants are always free.
1 adult volunteers per 6 children is no charge. Additional adults are $8.00 plus HST each.
